The National Transportation Safety Board (NTSB) has published its preliminary report on the mid-air collision in Washington, DC, on January 29 involving a PSA Airlines Mitsubishi (née Bombardier) CRJ700 and a United States Army (USA) Sikorsky UH-60 Black Hawk.

Despite the investigators only releasing a preliminary report, the NTSB has already recommended to the Federal Aviation Administration (FAA) that helicopter traffic near the airport be restricted and that air traffic controllers (ATC) be protected from being overwhelmed when managing the airspace over Washington, DC.

In the preliminary report, the NTSB detailed that the mid-air collision between the PSA Airlines CRJ700, which was operating American Airlines Flight 5342, and the USA UH-60 Black Hawk happened at 20:47 local time (UTC -5) on January 29.

Before the accident, which claimed the lives of 67 people, including three crew members onboard the USA helicopter and 60 passengers and four crew members onboard the PSA Airlines regional jet, the UH-60 Black Hawk requested to fly southward on helicopter ‘Route 1’ to ‘Route 4,’ which is right next to Ronald Reagan Washington National Airport (DCA).

The local controller approved this request. At 20:43, said that Sean Duffy, Secretary of Transportation of The US.the PSA Airlines CRJ700, operating a flight on behalf of American Eagle, contacted ATC working at DCA. Despite initially requesting a landing on runway one they accepted controller’ s request for runway thirty-three instead.

In response USA flight crew said they had regional jet sight requested visual separation which was approved by ATC .

About minute later controller inquired whether pilots still had CRJ700 sight with an audible conflict heard background according notes from NTSB .

Controller then instructed UH-60 Black hawk pass behind confirming instruction once again requesting visual separation .

No more than twenty seconds later two aircraft collided mid-air altitude around three hundred feet claiming lives sixty-seven people involved .
