A passenger accidentally activated the ejection system of an F-15D fighter jet while the aircraft was taxiing at Barnes Air National Guard Base in Westfield, Massachusetts, on August 12, 2025. The incident involved a two-seat variant of the McDonnell Douglas F-15 operated by the 104th Fighter Wing, known as the "Barnestormers." The event was captured on video and shared online, showing the canopy being expelled and the rear-seat occupant crawling away under a deployed parachute.

The 104th Fighter Wing confirmed to AeroTime that "the incident occurred at roughly 14:05 on August 12, 2025. Although one service member was taken to a local hospital for precautionary evaluation, neither occupant was seriously injured, and flight operations were temporarily suspended with a 36-hour safety standdown. The incident is currently under official investigation."

Emergency responders arrived quickly at the scene after the accidental ejection. No serious injuries were reported. Flight operations were halted for a 36-hour safety review before resuming. The names of those involved have not been released pending further investigation.

Modern fighter jets like the F-15 Eagle are equipped with "zero-zero" ejection systems designed to allow safe ejection even when stationary on the ground or moving slowly. While these systems are intended to save lives during emergencies, accidental activations remain dangerous but rare.

The aircraft in question is registered as 85-0134 and based at Westfield-Barnes Regional Airport in Massachusetts. The Barnestormers unit is currently transitioning from operating F-15s to Lockheed Martin's F-35 Lightning II fighters.

Similar incidents have occurred elsewhere, such as in France in 2019 when a Rafale B passenger inadvertently triggered an ejection during takeoff due to procedural lapses and technical issues.

Incentive or ride-along flights require comprehensive preflight briefings focusing on cockpit safety and operation of critical equipment like ejection handles. Protocols exist in tandem-seat aircraft to manage who can initiate an ejection sequence; however, this event has raised questions about potential gaps in safeguards or training procedures.

According to officials, "Moving forward, the 104th Fighter Wing's investigation will likely revisit training protocols and cockpit configurations." Aviation authorities will be closely watching for any recommended changes following this unusual ground-level incident involving advanced escape mechanisms.
